Academic Renewal Petition

Students who want to return to Iowa State University to pursue an undergraduate degree after an extended absence may request permission to remove one or more incomplete terms from future degree and GPA considerations. The renewal petition cannot be used for students who have graduated from Iowa State University to pursue a second degree, teacher certification, etc. This form can be completed online using Acrobat Reader 6.0 or later, then print it (remember to sign the form).

Change of curriculum/major; or advisor

Minor, Request or Cancel

Students will use the Minor Request form to request, change, or cancel their minor. This form can be completed online using Acrobat Reader 6.0 or later, then print it (remember to sign the form).

Multiple Curricula or Multiple Degrees

Students will use this form to declare, revise, or cancel multiple curricula or multiple degrees. This form can be completed online using Acrobat Reader 6.0 or later, then print it (remember to sign the form).

US Diversity and International Perspectives Course Proposal

Departments use this form to propose courses for the U.S. Diversity and/or International Perspective Requirement. Review the guidelines and submit the U.S. Diversity and International Perspectives Course Proposal form to the college curriculum coordinator for approval. The form is then forwarded to the Faculty Senate Curriculum Committee. Complete this form online using Acrobat Reader 6.0 or later, then print it.

Withdrawal Request

Per the student’s request, the “Request for Withdrawal” form is initiated and submitted by the student's college office to the Office of the Registrar. The student is withdrawn from all courses based on the withdrawal date on the form, and tuition and fees are adjusted, if appropriate according to established policies approved by the Board of Regents, State of Iowa.
